To comply with section 3.1.4 of the Cross-Connection Control Policy Handbook (CCCPH), each public water system (PWS) must develop a written Cross-Connection Control (CCC) Plan. 

As part of that plan, we are required to conduct a hazard assessment. To complete our hazard assessment, we are asking all residential and commercial customers to fill out the Cross Connection Control Hazard Assessment
